Jeanine: Don’t know where to start. My son has hydrocephalus, eye problems, and was just diagnosed with ODD, ADHD, and Mild Neuro cognitive disorder and might be on the spectrum but the doctor needs to look at the test results.

Have your child’s physician write a summary letter about his disabilities (e.g. “Johnny Smith has been evaluated in my office and identified as having the following diagnoses: hydrocephalus, (name of specific eye problem), Oppositional Defiant Disorder, ADHD, and Mild Neurocognitive Disorder resulting in deficits in (memory, attention, whatever).” Have the doctor sign the letter and give to you
Then, send the principal via REGISTERED MAIL WITH RETURN SIgNATURE a request for a Full Initial Individual Evaluation (FIIE) from the school (examples online) and include a copy of the physician’s letter. The school has 10 business days to respond in writing as to whether they intend to accept the physician’s report, do additional testing, or deny your request.
